Help with college recommendation outline?

I have two recommendation outlines to give to two teachers of my choice for college. On the outline it asks me to fill out questions about myself. I’m stuck on the adjectives about myself I am suppose to list. I am a smart student this is just really stumping me! I put others feelings and ideas before mine. I am very proactive for the future. I set realistic goals for myself. Any adjs ideas? Thank you
