Why can't I get favicon.ico to show up as my website tab and bookmark icon?

I honestly can’t believe I am even asking this, but I have tried everything and still no joy. It is something I have done 100’s of times, but this time am having no luck.

Basically, I am trying to make a small icon for my website to use as its tab icon and bookmark icon, just like Fluther, youtube, godaddy and all the other big sites do.

I have created a 16×16 favicon.gif graphic and converted it (not renamed it) to favicon.ico.

I have uploaded it to the root folder on my hosting, and even though it is not necessary with modern browsers, I even added a link in the <head> of my site to the favicon, just in case that was the problem.

I have tried creating the .gif in just 16 colors, I have tried with bmp, jpg, png and other formats.

I have tried everything I can think of, but for some reason just can’t get my favicon.ico to show up in my website tab.

I even tried deleting robots.txt just in case I was somehow disallowing it from being seen, and have even tried bookmarking my own site, closing and reopening the browser, try different browsers, and everything and anything else I could.

Why won’t my website display my favicon?

This should be simple stuff, I feel like a pianist who is asking “what are piano keys?”

Excuse me for only reading the last sentence of your detailed question, but they are the black and white things which, after you pressed them, will make that a variety of sounds will emerge from the big black, often wing shaped, wooden instrument.~

Unfortunately I don’t have an answer for the icon question, other than a very obvious one: does the code (I think that that is what it is called?) refer to the correct position on your computer like C/poison/images/website instead of C/poison/music/website?
