What's the deal with student/teacher relationships in college?

Not related to my current personal situation (although perhaps for future reference… just kidding!)
A couple of answers to the recent Have you ever hit on a teacher? mentioned times where both the student and the teacher were adults.
Have there been any situations where this sort of relationship has become problematic? Is it something universities have official guidelines on? I have no idea.
