MLA formatting a paper?

My professor specified that he wanted us to have a title page: Therefore, should I have my name/etc. at the top of the first actual page, like in a normal MLA paper?

If no other format was specified and there is no book of style that’s standard for the school/class, then I would assume so. And I join @Kayak8 in welcoming you to Fluther.
