How do you direct-dial an international number (Europe) from a phone in the USA?

So if someone gives you a European number such as +45 1234 56 78 or 0045 1234 5678, what would one actually dial from the USA?

start with 011. If you are in a hotel or a business that has a special phone system, start with 9 then do the 011. Then the country code (45) and the rest of the number.

and if you are on a cell phone, you may have to hold down * or zero (on my iphone) to get the plus symbol, then you can just start with the country code.

uk is 011 then 44 then miss off the first zero from the area code.
