Help me word a consent form?

Ok so I found a way to get out of an hour long car ride with my ex to get my son’s passport…
I just need to bring a notarized consent form from him but I don’t know how to word it… so far I have

“I, [dirty; cheating; ex,] give permission for my son to obtain a passport. His mother [beautiful, awesomely great, person I never should have cheated on…aka krose1223] has my permission to complete necessary forms without my presence.”

blah blah blah. It sounds so weird and dumb to me. HEEEELLLLLLP!!

Just keep it short and to the point, while making sure you meet all the requirements. Honestly, what you have so far sounds good to me. If he needs to mention why he can’t be there, just add that in. Your draft doesn’t sound weird or dumb to me.

Legalese always sounds silly. Don’t worry about it! As long as it gets the job done, it’s fine.

No need for notary, but might make it seem more legitimate with a seal on it. The notary can also advise you on wording if they are nice, and if you are beautiful like you say.
