How To: High dynamic range imaging

I am pretty experienced with Photoshop, but I was wondering if anyone could provide me with a step by step “how to” for producing HDR images from start to finish. Complete layman’s terms are not necessary, but please, make it understandable to a wide audience.

If doing this post processing then I will change the exposure levels in raw before importing them into PS. I have on a few occasions taken bracketed exposures, which is in my opinion, the way to go, but not possible with some scenery, particularly running water.
