What happened in the Dark Knight?

I feel really stupid for asking this question, but I want to know.
What happened with Rachel in the movie? I know she died, and got blown up, but who kidnapped her? I watched it for the first time on TV, so they probably cut out some parts. Why was Dent going after the cops?

Rachel was kidnapped by the Joker, as was Dent. When Batman tortured him, he gave they the addresses for them both, but switched who was where. Batmans saved Dent, but the police were slow to save Rachel.
Dent, as Two-Face, began killing the crooked cops who helped the Joker kidnap Rachel.
