Why does this itch where the sun doesn't shine persist? (NSFW)

I’ve been avoiding this for quite a while now because A) it’s embarrassing and B) well – it’s embarrassing.

I think it’s been 2 years now since I’ve had this itch near my anus that doesn’t seem to go away. At times after I put Neosporin or Vaseline or any other ointment it calms down. Then it flares up randomly and I’m like “Oh GOD I REALLY NEED TO SCRATCH IT RIGHT NOW” and excuse myself for the bathroom. At first I thought it was stress-related, because it seemed like the less I thought about it the less I needed to scratch. Then I thought that maybe I didn’t clean myself down there often enough? But even after I washed that area in the shower daily, the itch persisted.

I’m going to the doctor this Thursday for an unrelated note, and I’m thinking about bringing this up to him but I’m so goddamn embarrassed about it. I don’t know how I feel about an old man looking at my asshole. How do I even bring it up? What’s the technical phrase or term? “There’s an itch in my anus?” Just like that?

The only person who has looked at it is my girlfriend. She said it looks like there’s a cut there – a very slim cut. You don’t notice it right away – you have to really spread the cheeks apart to realize it’s there. There’s nothing that looks alarming – she took pictures of it. When the itch is incessant and I scratch really hard, then it bleeds a little.

Is this an anal fissure? But I read that those should have healed up by now. It also doesn’t hurt when I poop though. It only ever hurts when I scratch it too much. I also, on a last note, tried disinfecting it with isopropyl alcohol, which stung like hell.

And before you ask – No. I don’t do anything sexual there.
