How does Twitter work (see details)?

If I wanted to send a message to one user, and I posted on his timeline, would everyone who follows them be able to see it too? Does it have settings that would make it so that only they can see it?

If you send them a DM (direct message) it will go just to them. If you tag them then it’s on the general twitter feed.
