How far back in time do you have to go to practice medicine without a license?

In different countries? When were medical organizations founded? Any field.

Licensure in the United States and Canada started in the second half of the 19th century. But licensing physicians dates back to the Middle Ages in Europe, and even earlier in the Middle East.
