Any type of violence in a movie or on stage is choreographed to avoid the chance for injury to the actors. That means that any slap, punch, or fistfight is choreographed. Any use of a weapon is choreographed whether it’s a knife, a gun, or a sword. All falls are choreographed.

Yes, the swordfight in “The Three Musketeers” is choreographed.

I have been in many plays. Even the simplest slap is choreographed. A simple fall is choreographed. An incomplete fall is choreographed. Safety is the #1 priority on any stage or set.

Like @Hawaii_Jake said it is all play acting, I’ve got a friend, much younger, that graduated from a University known for sets, acting and singing. He is a trainer for fights and sword play at an singing and acting academy.
