What should I include for a resume submission for a game show?

I am trying to audition for a show on MTV. I have to include a resume, but I do not know what to put for it. Any help is great!

What game shows want from their contestants is not intelligence, but viewablity.
They want excited people that are fun to watch!
Include a link to a youtube video you post of yourself at a birthday party screaming your fool head off.
