Does anyone know of a free or cheap ap for Facebook fan pages that allows you to run contests and collect email address?

I herd about some changes that Facebook made months ago, and it wasn’t until I read your question did I look it up.

Facebook is now allowing contests by approval only, and once approved you must purchase a minimum $10,000 ad budget.

So to my understanding, even if you use an app, you still must get approved and pay Facebook. Sorry I did not answer your question directly.
