Did any workers get buried in the concrete when they were building Hoover Dam?

As a kid, I remember my mom asking my dad this question.
His answer was “Sure, but who would admit it publicly?”
Typical.
Anyway, I have always wondered if anybody actually did fall in and get buried when they were pouring concrete for Hoover Dam.
