Can somebody help me with my reccomendation dilemma?

I’m a senior in high school getting all geared up for the college application process which first begins with the teacher recommendation. I plan to major in Biology at college so I plan on asking my anatomy teacher for a recommendation and my math teacher from last year. Anatomy I have an A and my grade last year in math was a D (I know, I know). Now, I plan on asking one teacher that I struggled with, saw after school, emailed, etc; my math teacher is a great candidate.

My dilemma is the fact I do not want him lying on my recommendation expressing how I am a worthy student to attend __ university. I never outweighed anyone in the class but he knows I worked my butt off in that class. Now my question is; should I have him write the recommendation? If yes; I plan on typing him up something addressing my feelings about last years class before I permanently ask him. What should I say? I’m so scared!
