When doing the Epley Maneuver for vertigo do you use a 45 degree angle first?

Some videos show 45 degrees and some show looking completely to the side.

Also, if the dizzy is on both sides, do I cure one side first and then wait? The doctor I saw said she might have to repeat it a few times; would she have done it twice or three times on one side and then immediately switched to the other side? Or, back and forth alternating?

Background: I was in an accident, I had never had vertigo before the accident. Mine was extremely bad and on both sides. A doctor diagnosed me in the hospital, but then chose not to do the full maneuver because I was in so much pain. The diagnosis did half cure it, but it is getting worse again and I thought I would try to do the Epley myself with my husband.
