Why would you call a woman sir?

And why would a woman want to be addressed that way? Do they do this in the military if a female is of high rank?

I was watching one of my favorite shows, Castle, and this female detective called her female Captain, sir. Is it just a sign of respect? Why not ma’am?
I’m really curious.
