How do I write a letter of guardianship?

Recently I have not been very well emotionally I suffer from BP Rapid Cycling. What I would like to do is give ‘health’ guardianship to a person I trust should there come a time where I cannot speak rationally for myself. This would be used in relation to any health care and doctors dealing with my case.

Any tips on how to draft a simple letter in regards to this.
