How can I convert a music file (in the form of a MIDI) into sheet music for a music box?

Okay, simply converting music to sheet music is an easy Google search, that’s not what I’m asking…

I am trying to build a music box. I bought this: http://www.uncommongoods.com/images/product/20232_zoom1.jpg

Works very well with the sample song they provide, but for my own song I need to punch holes in the little sheet. Which is not a problem if I knew what I was punching.

I have a song in the form of a Midi. Not sure how to post the midi file here but the song is Drume Negrita (kind of popular) and when I pull it into garage band I get something that looks like this: https://www.evernote.com/shard/s286/sh/6adeb06a-96e1-4ab2-956a-1b7be9ff3d87/efcb427b838b517fbeab1a92c3768a04/deep/0/My%20Song%202.band.jpg

Which feels like I’m on the right track. But I need to somehow convert this into something I could punch holes into on this piece of paper. The music boc only plays in C I believe which is fine.

I found a site that will convert this midi to sheet music and I get this: https://www.evernote.com/shard/s286/sh/2870e460-2f23-4e00-bcf0-6295dab2ae68/bf012c440ef67e7c06317b3d06ff20f1/deep/0/mm1430177204539.385380406423-midi-page1.png.jpg

Not sure on the accuracy but I don’t even know how I’d begin translating that into holes on a sheet like this: http://www.markcolgrove.com/misc/The_Office_Music_Box_Strip.jpg

Anyone have any ideas?
