How did the Moon get there?

What is the best current scientific thinking on the origin of the Moon? Was it the result of material ejected from the Earth in an ancient collision with a large asteroid or planet? Could a large object colliding with the Earth actually punch completely through it and throw out a mass of super-hot material, similar to an exit wound on a human body, that would eventually become rounded off into the body we know as the Moon?
