They are covered by other high-risk insurance companies, the same that cover airplanes and yachts. Think of premiums in the $2k+ / month range here. With a very high deductible, $20k or more.

However, the requirements in most state (CA as a reference) you can exclude yourself from the liability auto insurance requirement if you place $150k in an escrow account to be used if you are in an accident.
