Is America causing it's own pain on healthcare costs?

It seems to me that the entire healthcare industry is more concerned with profits than with health or effectiveness. I have heard on NPR about a doctor in India who has managed to cut the price of open-heart surgery to $2000US, I have heard people actually worrying about reducing costs because that means that someone somewhere will make less money, and then there are just plain old ineffective (and potentially dangerous things like the overuse of antibiotics. I have also recently seen how Americans spend far more on healthcare and yet we don’t have the average life expectancy to match that expenditure. (Source)
Between things like those little tidbits and other observations throughout the years, I have to wonder how much of the healthcare problem is our own doing. If I were a little more cynical, I would almost say that it seems almost like our country is trying to exterminate the lower-income people! I know that’s not actually true, but I’m not sure that they could do so more effectively if they did try.
