How can I specify the page name for Twitter and Facebook sharing buttons on front page of website?

I have a duplicate of my latest blog post on the index page of my website. For the permalink blog entry there are Facebook and Twitter share and “like” buttons that share the post title when clicked. On the homepage, if I share the post, the shared tweet or Facebook post displays the title of the homepage. How can I change it to the post title without specifying the URL for the plugins on the homepage every time I write a new post?
