Any jellies well-versed in Adobe Reader (see details)?

I have been using an editable PDF (SSA-X2 D&D Character Sheet v3.0a18) for a couple years now without incident. Just recently, however, it has been giving me an error message that I can’t seem to get rid of. Every time I try to input information into an editable field, I receive this message:

*Warning: JavaScript Window -

Invalid date/time: please ensure that the date/time exists. Field [ date_modified ] should match format mmmm d, yyyy*

and it redirects me to the bottom of the 6th page in the document. I’ve edited every setting I can find, reset the sheet, and even reinstalled Adobe reader, and still the problem persists. Can anyone help shed some light on this problem?
