What are some of the more memorable outtakes from movies or TV that you can recall?

Primarily found during the closing credits in an ever growing list of comedy films. There have also been quite a number of TV shows chronicling the “bloopers” made down the years in that particular medium. So, with that in mind, either give an accurate commentary on those funny enough to be included. Or better still, post a link of any classic “cock ups” that tickled your fancy.
