How can I force an employer to give me a W2 for last year? I believe he was evading federal taxes.

I worked for an employer last year as a waitress.
I’ve tried several times to contact this person in pursuit of a W2 or at the very least a 1099. I had over 2500 in income before tips last year with this employer. I cannot get in touch with my employer and taxes are due soon. I believe this employer was evading irs federal taxes. I never received check stubs. He paid me in cash, and said he was taking taxes out of my money, so at the time I thought that he was doing everything legally. I’m not sure what to do. But I’m a single mom and need the income or I’ll receive no tax refund (which I desperately need) PLEASE HELP.

Will I get in trouble if I call and report that I think he may be doing things illegally? What can I do to still claim this job and get my taxes finished on time.
