When it comes to treating pneumonia does it matter what country you are in?

For instance, a person died of pneumonia in Puerto Rico. My question is, had the person been in the U.S. (citizen went to P.R. to visit or something?) would he have gotten better care?

i.e. would doctors try to do more? better tools perhaps?
