How much does one legally have to reveal to life insurance companies?

I currently have a life insurance policy with my job. But am leaving for health reasons.

So I have a broker working with me. They filled out the initial questionnaire. Which I answered truthfully.

I have an appt for a medical exam. I don’t know how thorough it will be. Am I obligated to reveal that I have a condition which will mostly likely objectively speaking and barring miracles less then 5 yrs be the death of me?
