Who is covered by the Q in LGBTQ?

It started out as LGBT, which seemed pretty inclusive. Then it changed to LGBTQ. I assume the q stands for queer, which I always thought to be a derogatory name for homosexual, and would be covered in a non-derogatory way by lesbian and gay.
