Can Someone Help Me.. Get A New Identity?

I am going to… “leave” my fathers house because he is extremely abusive and i want to change my name and get a new social security number… but i dont know how to because wouldnt u need id. so technically i want i am going to get a new Identification .. can someone please help me..
AND PLEASE dont tell me, you should tell someone or go live with your grandparents or try to talk me out of it because.. ive already done all i could and it still hasnt worked.. i already have my mind set.
