Can you tell me what the position of "Transition Coordinator" at a Community College would entail?

I have an interview for that position, and I’d like to figure out what’s up before I go in. I’m sure, by the title, that it has to do with transitioning HS kids in and graduates out, but does anyone know exactly what that means?
