Won't people start pleading insanity if we only go through with the death penalty if we are 100% sure?

One of the main objections to the death penalty is we make mistakes and innocent people die, and normally the counter argument to that is it should only go head if we are 100% sure. But please correct me if I’m wrong but won’t everyone just plead insanity because you can’t prove 100% someone is insane or not? And they will just get away with it?
The tests to determine is someone insane or not have been made by humans and imperfect, is insanity objective or subjective?
