What on earth is the title/artist of this song?

This song was also used in an episode of The Ghost Whisperer entitled Mended Hearts

I saw this episode last night and taped the credits hoping they could provide information, but my camera didn’t capture them clear enough to read. :(

Some lyrics are “You’re my heart, you’re my soul, help yourself, don’t let it get cold..”

I’ve been searching for this off and on since 2006.

I would be ever so grateful for any help in solving this musical mystery.
