Movie about Grim reaper?

Here’s what I remember:
1.He collects his victims souls and then brands them with a number.
2. there was a hospital where everyone in it was murdered (has something to do with the plot)
3. there is some really cool scene where this lady cheats death after she crashes her car? but is thrown off the highway moments later by the grim reaper, takes her soul.

Thats all I remember it came out maybe late 80s early 90s. anyone know?
