Would you please give me advice on writing a professional mini-biography?

I am an Audiologist employed by a large group of Ear, Nose & Throat Physicians. They are installing a video system in our waiting rooms with information for patients, and they want us to include a mini-bio with our educational and professional background, but also some personal information.
I don’t have much interesting, because I’ve been a broke single-mom struggling to overcome self-esteem issues from childhood abuse. As such, I’m not too concerned about it sounding pretentious, but I don’t want people to roll their eyes when they hear it.
As a patient, what would you want to know about the person you are waiting to see? As a Professional, how would you market yourself to your clients?

You can be very generic about your personal info. Born in a manger, 2.1 children, living in Ourtown for 17 years, member of the Local Grange. Then the rest can all be professional info. You needn’t publish the details of your treatment at the hand of an abuser.
I think the best thing to do is take the best from several samples. Here is one from your field. Here is another.
Finally, this includes one approach to personal information and this shows a different approach with the person’s philosophy.
So, basically, your training and a brief background of your work and years of experience. Then, you can adds any awards or publications or professional memberships.
I agree with @pdworkin about keeping the personal info limited. @hearkat has lived in our community for x years with her child.
I like the idea as a consumer of reading someone’s approach or philosophy about their profession.
@Marina: I don’t even live in the community! One office is 44 miles from home, the other is 56. Yes, those are one-way distances!
I am but a humble clinician… I have no awards or publications, just a bunch of patients who love me… but I can’t exactly say that!
