How do I write a letter of guardianship?

Recently I have not been very well emotionally I suffer from BP Rapid Cycling. What I would like to do is give ‘health’ guardianship to a person I trust should there come a time where I cannot speak rationally for myself. This would be used in relation to any health care and doctors dealing with my case.

Any tips on how to draft a simple letter in regards to this.

Go to a hospital and ask them to get you a form for a medical power of attorney. Ask them if it would be appropriate for your situation (I think it would be). All you need to do is fill out the form, and (I think) have someone witness it. Get more than one form because you’ll want to give one to the person you designate as well as one each to the hospitals in your area if it’s possible you could end up there inadvertently such as an accident.

Some hospitals might want you to fill out their own form but I can’t see why that should matter so much. You might need to make some phone calls before you go too far.

If you are going to have someone appointed medical power of attorney, you also might want to have someone else appointed to handle your finances if you become incapacitated. It’s best if it’s not the same person (to protect both you and them from fraud). For this second part, you would need an attorney.

Looks good, although I didn’t read it. I am in the US, and I was speaking out of my own experience. The part about having separate people for medical and financial guardianship is especially valid due to concerns of fraud.

I agree with everything @snowberry said, but I recently had the paperwork done from the hospitals own forms which had spaces to add specific edits and addendums if necessary.

They had a notary there and they gave me plenty of copies.

They did mention financial part so I went to the business office there to sign, and if I signed one I had to put a specific starting date or from this date forth… etc. Also it seemed like it would only be useful for that particular billing department. So an attorney or some other form must be used.. I haven’t looked into it further.
